چکیده

Introduction Albeit defined as an inspiratory muscle, the diaphragm interacts with the respiratory system also during expiration. The expiratory load on the diaphragm derives from lung volume and hence, possibly from lung collapse. We hypothesized that the diaphragm has a braking effect during the whole expiration, preserving lung patency during SB in collapse prone lungs. We investigated this hypothesis by measuring the expiratory electrical diaphragmatic activity (EAdiexp) in the modulation of expiratory flow (V’exp) in a porcine model.

Diaphragmatic neuromechanical coupling and mechanisms of hypercapnia during inspiratory loading

We hypothesized that improved diaphragmatic neuromechanical coupling during inspiratory loading is not sufficient to prevent alveolar hypoventilation and task failure, and that the latter results primarily from central-output inhibition of the diaphragm and air hunger rather than contractile fatigue. Eighteen subjects underwent progressive inspiratory loading. By task failure all developed hype...

Diaphragmatic electrical activity: a new tool to assess lung hyperinflation?

447 September 2014 “ EXPIRATION and slow decrease of light together reaching minimum together ... in about 10 s and immediately cry as before. Silence and hold for about 5 s.” In the script for his late play “Breath,” Samuel Beckett conveys in sound and light his awareness of long and vital expiratory times. His personal experience with emphysema might have provided some of the inspiration. In ...

Electrical and mechanical activity in the human lower esophageal sphincter during diaphragmatic contraction.

To determine the effect of contraction of the diaphragm on the lower esophageal sphincter (LES) pressure, we studied eight healthy volunteers during spontaneous breathing, maximal inspiration, and graded inspiratory efforts against a closed airway (Muller's maneuver). Electrical activity of the crural diaphragm (DEMG) was recorded from bipolar esophageal electrodes, transdiaphragmatic pressure ...

Effects of airway occlusion on breathing muscle electromyogram signals, during isoflurane anaesthesia, with and without the effects of fentanyl and hypercapnia.

BACKGROUND Expiratory muscle action is prominent during anaesthesia and can impair lung function. This activity is exaggerated by the use of opioids. Airway pressure during occlusion of expiration would be a valuable measure in the study of expiratory muscle activation. However, this would only be valid if the imposed occlusion did not itself alter muscle activation. This possibility can be che...

The Effect of Caffeine on Diaphragmatic Activity and Tidal Volume in Preterm Infants.

OBJECTIVE To determine the effect of caffeine on diaphragmatic activity, tidal volume (Vt), and end-expiratory lung volume (EELV) in preterm infants. STUDY DESIGN Using transcutaneous electromyography of the diaphragm (dEMG), we measured diaphragmatic activity from 30 minutes before (baseline) to 3 hours after administration of an intravenous caffeine-base loading dose in 30 spontaneously bre...
